Burnley East Primary Care Network (PCN) aims to enhance healthcare services by offering a range of additional roles that work in conjunction with your GP surgery. These roles include:

Social Prescribers: They help patients access non-medical support, linking them to community activities, welfare support, and other services to improve overall well-being.

Clinical Pharmacists: They manage medication, ensuring its safe and effective use.

Paramedics: They provide support to care home residents, offering weekly ward rounds, care planning, and ongoing care.

Health and Well-being Coaches: They aid patients in making lifestyle changes such as improving diet and exercise to manage and prevent health issues.

Care Coordinators: They help coordinate care for patients with complex health needs, ensuring they receive appropriate services.

Mental Health Practitioners: Providing mental health support to complex mental health patients, providing them with the tools they need to effectively manage their own needs.

Nurse Associates: They work alongside general practitioners to provide nursing care in the community, with a focus on the prevention and management of chronic conditions.

Advanced Nurse Practitioners: They support patients in care homes, providing weekly ward rounds, care planning, and ongoing continuity of care.

Physiotherapists: Our First Contact Physiotherapists provide assessments and management plans for a wide range of musculoskeletal issues.

Care Home & Over 75s Service
The care home & over 75s team, consisting of care coordinators, frailty nurse, Nurse associate and advanced practitioners, plays a crucial role in providing quality care to the elderly and vulnerable residents. They work together to ensure that residents receive the appropriate care and support they need. The team conducts weekly ward rounds as part of the NHS Enhanced Health in Care Homes (EHCH), which helps to identify any health issues and provide timely interventions. Their dedication and expertise ensure that the residents receive the best possible care and support.
Health and Wellbeing Team
The health and wellbeing team consisting of social prescribers, health and wellbeing coaches, and nurse associates provide comprehensive support to patients. They can offer outreach health and wellbeing clinics and support, as well as work with patients to develop personalised care plans. Social prescribers connect patients with community resources, while our health and wellbeing coach provides guidance on lifestyle changes. Our Nurse associates can assist with provide enhanced health checks at outreach locations. Together, this team can help patients achieve better health outcomes and improve their overall wellbeing.
