Complete Care Community
Burnley East Primary Care Network is one of 65 site nationally taking part in the complete care communities project, this project aims to tackle health inequalities in areas of high deprivation. Burnley East are providing our Over 65s population with a comprehensive health, wellbeing and digital capabilities assessment to support them living and agenig well.
Diabetes Workshops
Our project aims to increase the number of patients from south Asian backgrounds who receive the EMPOWER diabetes education programme, increase the number of patients who access Healthy Living for People with Type 2 Diabetes by supporting digital inclusion. We endeavour to equip local community leaders with Making Every Contact Count (MECC) training so they can encourage and support peers to participate in both EMPOWER and the Healthy Living Programme. The PCN will also provide group peer support groups within the community to educate, guide and support patients living with Diabetes.
Tackling Inequalities and Awareness
The PCN provides engagement events, focus groups , and population questionnaires to ensure we fully understand and can mitigate any barriers to services, awareness of prominent health issues and deliver services tailored to our population.
Living Well Checks
Living well checks are comprehensive health and wellbeing assessments carried out at Outreach locations at local community, voluntary and faith venues. The assessments include support for finances, social issues, housing and a full health check.
