What is a PCN?
PCNs build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home. Clinicians describe this as a change from reactively providing appointments to proactively caring for the people and communities they serve.

Burnley East PCN is a collaboration of 6 GP surgeries but also works in partnership with the local hospital trust (ELHT), local council for voluntary services, statutory organisation, local district council and other organisations to bring innovation in the way we provide healthcare.

A primary care network consists of a team of healthcare and administrative professionals who work together to provide comprehensive and coordinated care to patients. Together, they work to ensure that patients receive the right care at the right time, and that their health needs are met in a timely and effective manner
